What We Do #1: Release of Information

Did you know that Rio Records Service, Inc., offers RELEASE OF INFORMATION SERVICE?

Rio Records Service offers release of information services to local Hospitals, Clinics, and Private Practices and in most cases does so at NO cost to the contracting facility.

Release of Information is the process that begins when a Patient, Legal Representative of the Patient, Insurance company, Doctor, Home Health Agency or Court Order/Subpoena presents to the medical facility a signed and valid Authorization to Disclose Health Information form.

Having the information on that form validated the process begins to fulfill the request.

How is the information on the Authorization to Disclose Health Information form (the request) validated?

These are a few things we look at to validate the request.

• For Walk-in requests
     1. The facility you are at is properly named as the facility given permission to release information.
     2. Patient Name is properly spelled and matches the accompanying copy of Identification Card
     3. Patient Date of Birth matches the accompanying copy of Identification Card
     4. Patient Signature matches the accompanying copy of Identification Card
     5. Patient Dates of Service match the actual dates on the official record
     6. Specific information requested is available (for example, lab work was requested not done as         only action on date of service was office visit)
     7. Unless otherwise stated the date of signature is NOT greater than 180 days from the date request was presented for processing.
     8. Depending on your jurisdiction a non-refundable research fee may have to accompany the request before work can begin.

• For Requests submitted via US Postal Service
     1. The facility you are at is properly named as the facility given permission to release information.
     2. Patient Name is properly spelled and matches the Name on Facility's patient record
     3. Patient Date of Birth matches the DOB on Facility's patient record
     4. Patient Signature matches on Facility's patient record
     5. Patient Dates of Service match the actual dates on the official record
     6. Specific information requested is available (for example, lab work was requested not done as
         only action on date of service was office visit)
     7. Unless otherwise stated the date of signature is NOT greater than 180 days from the date request was presented for processing.
     8. Depending on your jurisdiction a non-refundable research fee may have to accompany the request before work can begin.

Once validated the process to fulfill the request can really begin. Again, depending on your local jurisdiction a fee for the records may be incurred.

***ATTENTION: If you live in one jurisdiction and request records from a facility in a different jurisdiction the facility's jurisdiction will be the one used to determine if fees for copies of the records will apply.

ADDITIONALLY, there are other factors that may exist and depend solely on the requested actions by the issuing requester. Such situations are handled on a case-by-case basis.
